We are here to help with Hospice In-Home Care
The hospice service’s team of health care professionals will work with the patient’s primary caregiver (usually a family member) to provide care and support 24 hours a day, 7 days a week. We want to ease the burden of you and your family caregivers. We closely with everyone involved in your hospice care to supplement care services when needed. As a result, your hospice home care plan is based on the patient’s specific needs and preferences. We stay in contact communication with the primary/family caregiver – as they continue to be the main person to help make decisions for the patient, throughout the period of hospice (end-of-life) care.
Our hospice care at home can also provide much needed respite relief and support for family members who are in the role of caring for their loved ones. Steadfast Home Care is able to temporary care to provide family caregivers with welcome breaks, from the full-time care they have been providing.
