We are here to help with Hospice In-Home Care
The hospice service’s team of health care professionals will work with the patient’s primary caregiver (usually a family member) to provide care and support 24 hours a day, 7 days a week. We want to ease the burden of you and your family caregivers. We closely with everyone involved in your hospice care to supplement care services when needed. As a result, your hospice home care plan is based on the patient’s specific needs and preferences. We stay in contact communication with the primary/family caregiver – as they continue to be the main person to help make decisions for the patient, throughout the period of hospice (end-of-life) care.
